Children’s Long-Term Support ForwardHealth Portal Welcome and PIN Letters
The Department of Health Service (DHS) mailed you two letters: a welcome letter and a letter with your PIN. These letters were sent to your billing address during the week of January 20, 2025.
During the week of January 27, DHS will send the same two letters to each of your service locations with information needed to log in to the ForwardHealth Portal.
Log in to ForwardHealth Portal
The welcome letter has the Children's Long-Term Support (CLTS) Medicaid ID (MA ID) number assigned to each of your service locations, if applicable. The other letter has your PIN to log in to the portal. Each service location will receive a welcome letter with a corresponding PIN letter. You will need these letters to successfully complete your portal registration.
Please complete your portal registration before the May 2025 transition. Additionally, if you would like to participate in Submission Sandbox testing you will need your MA ID and PINs to set this up. Read our recent message about Submission Sandbox testing!
Portal functionality
The portal functionality will include, but is not limited to:
- Direct entry of claims into the portal.
- The ability to copy a claim in paid status to a new claim for editing.
- Creating claims from authorizations.
- Viewing CLTS authorizations.
