OPPA: Public Health Infrastructure (PHI) Grant Accreditation Funding Support Interest Survey

This message is being sent to local public health officers, Tribal health directors, and key DPH staff.

PHI Grant funding to support voluntary public health accreditation costs is coming soon!

Accreditation Funding Support Interest Survey responses are due by 11:59 p.m. on Friday, December 6

The PHI Grant Management Team established a procurement relationship between the Wisconsin Department of Health Services (DHS) Division of Public Health and the Public Health Accreditation Board (PHAB). This relationship will allow the PHI Grant Management Team to eventually execute a contract with PHAB that will cover fees associated with local and Tribal health departments (LTHD) accreditation, reaccreditation, and accreditation readiness efforts. This procurement relationship with PHAB was established in large part due to input from a focus group of local public health staff. 

Your agency’s response is critical to establishing the budget and agreement with PHAB, which will be built directly from the interest survey data . All LTHDs must complete this interest survey to indicate interest in pursuing their agency’s unique next step toward achieving or maintaining public health accreditation, regardless of their current status.* The “Potential fees to be covered” section details what the next step might be for your agency. It should take approximately five minutes to complete the interest survey.

*We will add a reminder in the PHI Grant November Monthly Newsletter. However, we will assume agencies that have not completed this survey by the December 6 deadline are not interested in these funds.

The following lists include links to PHAB webpages and documents that further explain the accreditation-related programs and their associated fees that may be covered for LTHDs through this funding.

Accredited and reaccredited agencies:

  • Annual fees
  • Reaccreditation application fees
  • Accreditation Readiness Assessment (Note: We recognize that some agencies that were accredited through a previous version of PHAB's standards and measures may wish to complete the Readiness Assessment to ensure that they are in line with the latest requirements.)

Important notes

  • Your agency's response to this survey is not binding. This information is needed for the PHI Grant team to budget accordingly and to ensure that we are providing this support as equitably as possible to all LTHDs.
  • Neither the PHI Grant program, nor the Division of Public Health, plan to require accreditation at any point in time.
  • Our goal is to allow all agencies to take this next step. However, if there is great interest in the PHAB funding from LTHDs and we are limited by the availability of funds, we will prioritize costs to be incurred by agencies that are not yet accredited.
  • LTHDs might have another opportunity in the future if now is not the right time to pursue another step toward accreditation. We hope to offer this opportunity again, possibly around June 2026.
