2025 State Health Assessment (SHA) Qualitative Data Sharing Request

This message is being sent to local public health officers, Tribal health directors, local public health nurses and staff, and key DPH staff.

2025 SHA development begins

The 2025 State Health Assessment (SHA) development process has begun! This SHA aims to be the most widely representative assessment yet, grounding itself in the voices of Wisconsinites from all walks of life. The SHA is successful when it represents of the experiences of folks from all parts of our state. This positions us better to leverage the SHA to advocate for policies and resources that effectively address the health and well-being issues raised.

Your work at local and Tribal health departments can help ensure the voices of your community members are reflected in the SHA. You have trusted relationships in your communities that facilitate honest and open sharing.

Our ask for you: Would you be willing to share any community conversation input, survey results, or other qualitative data from a community health assessment (CHA) or similar process from 2023 or sooner? We’re happy to receive de-identified, unprocessed data, summary themes, final reports, and anything in between—whatever format is the lowest burden for you. This may be information that was or was not prioritized in CHAs or other publications. All data will be aggregated regionally and statewide, and not attributed specifically to your health department unless you would like it to be.

If you have any questions, or have data to share, please contact Julia Nagy (julia.nagy@dhs.wisconsin.gov), the population health improvement strategist for the state health plan. You can also visit the DHS State Health Plan webpage to learn more.
