New Student Nurse/ Graduate Nurse Procedure for the Wisconsin Nurse Aide Registry

The Division of Quality Assurance is sending this message as part of our commitment to Collaboration, Innovations, and Quality.

Student Nurse and Graduate Nurse applicants for the Wisconsin Nurse Aide Registry will soon be able to apply online! The application, available as a fillable form linked on the Wisconsin Headmaster and DHS Nurse Aide Training and Registry websites, aims to be more user-friendly and speedier than the current paper process.

The Department of Health Services (DHS), Division of Quality Assurance Nurse Aide Training and Registry team developed the online application in partnership with the registry management and testing company, Headmaster. Applications received after July 1, 2024, will be processed by Headmaster through their online system. Nurse aide certification renewals will continue to be processed by DHS.
