Apply for a Wisconsin Medicaid HCBS Grant by March 21
$15 million in funding available to support quality and innovation in HCBS
The application for the latest Wisconsin Department of Health Services (DHS) Medicaid home and community-based services (HCBS) grants opportunity is live today! Eligible organizations may apply for $25,000 to $1 million in funding. DHS will select projects that positively impact people receiving HCBS in communities across Wisconsin.
Applicants will use GrantsConnect, an online application portal, to determine eligibility and submit applications. Note: If you applied for a grant in Round 1 and didn’t get funding, we encourage you to apply again. Virtual office hours
