New MDS 3.0 Section Q Referral Process to Local Contact Agencies (LCAs)
A new Nursing Home Minimum Data Set (MDS) 3.0 Section Q Referral and Non-MDS Q Referral form (F-00311) is now available. Nursing homes can use the new system to make referrals to Aging and Disability Resource Centers (ADRCs). The current referral system will be discontinued on December 1, 2022. The new referral system includes instructions for nursing homes about how to fill out the form and where to send it. In addition, training for nursing homes and ADRCs is being developed and will be available soon. The RAI/MDS 3.0 Information webpage will be updated to reflect the new process. Beginning December 1, 2022, ADRCs will receive referrals from nursing homes by email or fax. Referrals will be made based on the resident’s county of residence.
