Apply for a Wisconsin Medicaid HCBS Grant—Application Open Through September 12

$30 million in grants available to support quality and innovation in home and community-based services

The application for the Wisconsin Department of Health Service’s (DHS’s) new Medicaid home and community-based services (HCBS) grants opportunity is live today! Eligible organizations may apply for $25,000 to $2 million in funding. Funded projects will positively impact people receiving HCBS in communities across Wisconsin.

Applicants will use GrantsConnect, an online application portal, to determine eligibility and submit applications. Below are some useful resources on the grant opportunity and how to improve your application.
