Effective June 22, 2022, the Division of Quality Assurance Southern Regional Office (SRO) is relocating to the Department of Health Services Central Office at 1 W Wilson Street, Room 455, Madison, Wisconsin, 53704. This move affects the Bureau of Assisted Living (BAL) and the Bureau of Nursing Home Resident Care (BNHRC).
