COVID-19 Vaccine Distribution Planning Survey: Children under 5 Years
Pending approval by the U.S. Food and Drug Administration (FDA), we anticipate that beginning in June, vaccinators will be able to place orders for new COVID-19 vaccine products for children 6 months through 4 years of age as they undergo review and potential approval for use.
Of note, administration of the vaccine will follow the same trajectory as other recommendations and is dependent upon an Advisory Committee on Immunization Practices (ACIP) recommendation and approval by the Centers for Disease Control and Prevention (CDC). The Wisconsin Department of Health Services (DHS) urges you to complete the COVID-19 Vaccine Distribution Planning Survey: Children under 5 Years as we plan for vaccinating Wisconsin’s youngest children.
This survey is intended to:
- Measure and map intent to vaccinate this new age group
- Inform the DHS vaccine distribution plan
- Proactively identify any potential gaps in access to these new vaccine products
Please complete and submit one survey per vaccination site by Wednesday June 1.
