This is an amended message to clarify that the templates available on SharePoint are preferred, but not required for reporting on the objective(s) you've chosen for your consolidated contract activities.
Immunization Program Consolidated Contracts
This is a reminder that your Consolidated Contract End-of-Year Report for the objective(s) you've chosen is due to your regional representative by February 28, 2022. The preferred, but not required, template to use is available on SharePoint:
Local Health Departments:
- Template for 24-month Up-to-date Objective
- Template for School Objective
Tribal Health Clinics:
- Template for 24-month Up-to-date Objective
