WEAVR Request Procedure Change

This bulletin is being sent to these lists: Division of Quality Assurance: Notifications and Updates, Assisted Living Forum, Local Public Health Officers, and Healthcare Emergency Readiness Coalition Coordinators.

Starting Monday October 18, 2021, requests from the Wisconsin Emergency Assistance Volunteer Registry (WEAVR) should be sent directly to the Department of Health Services by email.

Please complete the WEAVR Request form and send it to: DHSWEAVRMail@wi.gov. Forms will be reviewed Monday through Friday, 8:00 a.m. to 4:00 p.m. A request for volunteers will be sent to the WEAVR registrants, and after 24 hours an email will be sent to the requester with the names and contact information of those who volunteered.

For assisted living and skilled nursing facilities, please consider contacting the DHS Rapid
Assistance and Support Team (RAST) at: dhsrast@wi.gov. RAST calls are not regulatory reviews. They are an opportunity for stakeholders to come together to discuss the current outbreak situation, develop a customized, multidisciplinary approach to your challenges, and connect you with a technical assistance support system.
