5 Days Left: $100 COVID-19 Vaccine Reward Program
There are just 5 days left to be eligible for the $100 COVID-19 Vaccine Reward Program. Any Wisconsin resident who gets their first dose of COVID-19 vaccine between August 20, 2021, and September 19, 2021, is eligible to receive a $100 U.S. Bank Rewards card. Registration for the program will remain open until September 30, 2021, but only those who receive their first COVID-19 vaccine dose within the program window are eligible for the rewards card.
We are asking that you help get the message out to your community by sharing the social media graphics, printable flyers, and posters we have available on the DHS COVID-19 Vaccine Partner and Vaccinator Resources webpage. Resources are now available in multiple languages.
We also ask that you, as a Wisconsin COVID-19 vaccinator, consider encouraging patients to register for the program during their 15-30 minute observation period, make outbound calls to folks to who aren’t vaccinated to inform them of the incentive, and enter all vaccination information into the Wisconsin Immunization Registry (WIR) as quickly as possible to ensure there aren’t significant delays when trying to verify vaccinations for the reward.
We hope this program will help us get more people vaccinated in Wisconsin. Everyone is eligible for the $100, as long as they:
- Are at least 12 years of age
- Live in Wisconsin
- Get their first COVID-19 shot from a vaccine provider in Wisconsin, Michigan, or Minnesota between August 20, 2021 and September 19, 2021
- Request their $100 U.S. Bank Rewards Card by September 30, 2021 at wisconsin.gov or by calling 844-684-1064 (language support is available)
As a reminder, proof of insurance, citizenship, or ID are not required to receive the reward.
