Replacement FoodShare benefits are available to FoodShare members living in Brown, Columbia, Grant, Iowa, Milwaukee, Monroe, Outagamie, Rock, Sauk, Walworth, Waukesha, and Waushara counties, who lost food purchased with their benefits due to severe weather and power outages that occurred between August 7 and August 11, 2021.

To request replacement FoodShare benefits, members must fill out the Request for Replacement FoodShare Benefits form (F-00330) and submit it by fax, mail, phone, or in person to their local agency by August 31, 2021.
