The Fit Testing and Respirator Survey is now available online.
This is a one-time survey meant to assess respiratory protection needs and provide resources as available. This survey is open to all health care entities in Wisconsin, including local health departments and tribal health centers, veterinarians, and community clinics. The information provided will be used to connect agencies with available resources. Resource requests will be filled according to current inventory levels.
Please complete the survey no later than Friday, September 10.
