Vaccine Returns
There have been some questions about which vaccines need to be returned and which vaccines need to be directly reported to WIR as wastage. The following information is for VFC and Vaccines for Adults (VFA) providers only and does not include instructions for COVID-19 vaccines.
All VFC and VFA state-supplied vaccines that are spoiled or expired must be returned following the guidelines of the VFC and VFA programs. The Request for Authorization to Return Vaccine form needs to be completed and submitted for all spoiled or expired vaccines. These are the vaccines that must be returned to McKesson. The instructions are also included with this form. Use this form for all vaccines that need to be returned except COVID-19 vaccines.
Influenza vaccine for the 2020–21 season is now expired and you should be completing the return form, submitting it to the VFC Team mailbox and waiting for that return label to be sent to you so you can return the vaccine to McKesson. At this time returns are taking 4–6 weeks to process. Please do not submit your return twice. You can also fax your return to 608-267-9493.
Wasted Vaccine
Wasted vaccine is defined as vaccines that cannot be returned. For example: Broken vial, open vial but not all doses administered; vials drawn into syringe but not administered. These are vaccines that cannot be returned. You no longer complete a form for this type of loss. You now will enter that information directly into WIR as wastage.
