DHS Medical Stockpile Access Policy and Request Form
The Wisconsin Department of Health Services (DHS) Medical Stockpile is a cache of state-owned medical surge supplies, personal protective equipment (PPE), and vaccination clinic supplies. The supplies are available at no cost to hospitals, clinics, and other healthcare providers in the event that an incident or situation outstrips existing inventory of such materiel.
During the COVID-19 Response the State Emergency Operations Center (SEOC) sent biweekly surveys to county and tribal emergency managers and to local health departments and tribal health agencies, on behalf of DHS, as a mechanism to request and allocate PPE and vaccination supplies. The allocations were provided to the DHS warehouse for order fulfillment and to arrange transportation and delivery.
As the supply chain has been replenished and these supplies are now available for purchase, the SEOC has ended the biweekly survey process. WI DHS has developed the DHS Medical Stockpile Access Policy and Request Form for requesting materiel from the stockpile and will be processing these requests going forward. The DHS warehouse will continue to fulfill approved orders, as well as coordinate transportation and delivery.
The DHS Medical Stockpile may be accessed if the following criteria have been met:
- An emergency event has occurred (such as a mass casualty accident or pandemic).
- Requested items do not exist in local stocks or depletion of existing inventory is expected to occur and supplies cannot be obtained from other sources in the needed time frame.
- Your facility has exhausted all attempts to obtain PPE from other sources.
- Your facility is following the latest OSHA, CDC, DPH, or other applicable guidelines for the use of PPE.
- You are ordering what is needed, but not more than a two week’s supply. You may be able to reorder. Products are non-returnable.
