2020–21 Seasonal Influenza Vaccine
The 2020–21 Influenza Vaccines will be expiring starting June 30 through July 30, 2021. All VFC and VFA flu vaccines must be returned following the Immunization Program policy by completing the return form which can be found on our VFC webpage. Scroll down to "trainings, resources and forms" and within the dropdown you will find the "Request for Authorization to Return Vaccine" form and instructions. If you have other vaccines that have expired that need to be returned, you can add them to the same form. Do not include COVID-19 vaccines.
Due to the number of returns, please allow several weeks for your return authorization number to be returned to you. Do not resubmit your return.
