Request to Complete One-Time Survey for PPE, Fit Testing, and Educational Resources
The State Emergency Operation Center (SEOC), on behalf of the Department of Health Services and Wisconsin State Lab of Hygiene, is conducting a one-time survey to assess the current needs of Adult Family Homes (AFHs), Community-Based Residential Facilities (CBRFs), and Resident Care Apartment Complexes (RCACs) for PPE, fit testing, and educational resources.
If you identify that you do not have an adequate supply or reliable vendor source, you will have the opportunity to request PPE through the SEOC in the survey. Please note, the SEOC may not be able to fulfill all PPE requested due to available inventory and other considerations.
Please take a few minutes to complete the PPE, Fit Testing, and Educational Resources Survey.
The survey is due by 5 p.m. on June 30. Any responses received after this time will not be included in the one-time PPE allocation. Only one survey per facility will be accepted.
