Bureau of Assisted Living Quality Assurance/Improvement Survey – We want to hear from you!

The Wisconsin Division of Quality Assurance, Bureau of Assisted Living (BAL) invites employees of Assisted Living Facilities (ALFs) regulated by BAL to participate in an anonymous Quality Assurance/Improvement Survey. Assisted Living Facilities include adult day care centers, 3-4 bed Adult Family Homes, Community-Based Residential Facilities, and Residential Care Apartment Complexes.

This survey asks you to think about quality assurance and improvement efforts at your ALF and asks for your advice about assisted living quality assurance and improvement for the future. The purpose of the survey is to identify best practices in quality assurance and improvement. Every voice matters!

Who should participate in the survey?

  • Management Staff (e.g., licensee, regional staff, administrator, house/service manager)
  • Nursing Staff (e.g., registered nurse, licensed practical nurse)
  • Direct Care Staff (e.g., caregiver, certified nursing assistant)
  • Ancillary Staff (e.g., activities, dietary, facilities management, human resources)

Survey:

Please complete the BAL Quality Assurance/Improvement Survey by April 30, 2021. Answer the questions with respect to your individual perceptions and knowledge of quality assurance and improvement in ALFs. Please only respond to this survey if you are an employee of an ALF. The survey is expected to take approximately 10 minutes to complete.
