Enrollment Deadline of November 25, 2020 for Phase 1A Vaccinators

Health care providers that will vaccinate in Phase 1A, should enroll in the COVID-19 Vaccine program by November 25, 2020. If you cannot complete the enrollment process by this deadline, but, still plan to vaccinate in Phase 1A, please email us with your enrollment plans prior to November 25, 2020.

Will vaccinators not participating in Phase 1A still have an opportunity to enroll?

Yes. For providers that plan to vaccinate in later phases, enrollment is ongoing. However, over the next few weeks, we may prioritize processing paperwork for those who are vaccinating in Phase 1A.

Which entities should enroll for Phase 1A?

Entities which plan to vaccinate health care workers, including long term care staff, should enroll for Phase 1A.

Why is there a new deadline?

Given the recent news, we anticipate that two vaccine manufactures will be applying to the FDA for Emergency Use Authorization in the next few weeks. Therefore, we anticipate that we will have COVID-19 vaccine allocations available in Wisconsin by the end of 2020.

By Phase 1A providers enrolling in the program by November 25, 2020, we can create accurate vaccine distribution plans and ensure that all regions of the state will have vaccine.
