The Disability Determination Bureau Accepts Telephonic Signatures on Certain Forms

The Disability Determination Bureau (DDB) has updated its policy to allow for telephonic signatures on certain forms. As of August 1, 2020, members may verbally provide authorization via the telephone in lieu of a written signature for both the Medicaid Disability Application, F-10112, and the Authorization to Disclose Information to DDB, F-14014, if all of the following are true:

  • The member is an adult age 18 or older.
  • The member is signing on their own behalf.
  • The member has submitted an unsigned Medicaid Disability Application or Authorization to Disclose Information to DDB form.

Members must provide the authorization through the new Medicaid Disability Application, Addendum, F-10112A.

If members submit an unsigned Medicaid Disability Application or Authorization to Disclose Information to DDB form, their income maintenance or tribal agency will call them to collect the missing signature. Members cannot sign telephonically if a legal guardian, power of attorney, or other authorized representative is signing on their behalf. The legal guardian, power of attorney, or other authorized representative cannot sign telephonically.
