Wisconsin Chronic Disease Prevention Program Announces Funding to Expand Coordinated Systems of Care
Pathways Community HUB Model
A statewide request for partnership with up to $250,000 in awards is available to support the development of vital infrastructure for the expansion of community health workers and necessary systems of coordinated care to strengthen health equity.
The Wisconsin Chronic Disease Prevention Program (CDPP) announces the availability of up to $250,000 in funding to support the expansion of community health workers and coordinated systems of care through the Pathways Community HUB model. The funding opportunity is intended for Wisconsin-based, IRS tax-exempt 501(c)(3) organizations, community coalitions, or government agencies.
Applications must focus on building the evidence-based Pathways Community HUB model which assists in strengthening health equity, chronic disease prevention, and social determinant of health barrier reduction through partnerships and systems which connect at-risk populations to community health workers (CHWs/health care liaisons) and necessary health care, public health, and social service systems.
The award is divided into two opportunities:
Phase 1: HUB Network Investigation Grants—$5,000 (Up to 10 will be awarded)
Phase 2: HUB Network Activation Grants—$50,000 (Up to 3 will be awarded)
