The Department of Health Services, Division of Public Health (DPH) has issued the following memos regarding hospital and nursing home transfers:

The Guidance on the role of COVID-19 testing in decisions around transfers from acute care hospitals to post-acute and long-term care facilities provides recommendations for the role and performance of COVID-19 testing on hospitalized patients being considered for transfer to a post-acute and long-term care facility (PALTCF).

The Guidance on the transfer of hospitalized patients infected with COVID-19 to post-acute and long-term care facilities (PALTCFs) provides recommendations for the discharge disposition of hospitalized patients infected with COVID-19 who no longer require acute care and are ready for transfer to a post-acute and/or long-term care facility (PALTCF).

The Guidance on the disposition of medically stable post-acute and long-term care residents (PALTCF) with confirmed or clinically suspected COVID-19 infection provides recommendations for the management of the medically stable post-acute and long-term care facility (PALTCF) resident with confirmed or clinically suspected COVID-19 infection.
