CARES Act Contact Tracing Interest Survey

The State of Wisconsin has announced several funding opportunities for local and tribal health departments (LTHDs) to enhance their COVID-19 activities through the Coronavirus Aid, Relief, and Economic Security Act (CARES Act). The purpose of this funding is to support local efforts toward our shared mission of stopping the spread of this virus with the "Box It In" strategy of testing, contact tracing, and isolation and quarantine.

Contact tracing is an essential piece of the "Box It In" strategy. The contact tracing model estimates scopes of work for disease investigation, contact tracing, and monitoring which is used to allocate resources. In order to estimate resources, all LTHDs are requested to complete the following survey to clearly indicate the scope of work that the agency is willing to complete.

In order to receive the calculated funding amount, based on the estimated scopes of work, each LTHD must select one of the three options. Each jurisdiction will have the option of electing whether they want to:

  1. Conduct all three elements of contact tracing (disease investigation, contact tracing, and monitoring) on their own;
  2. Partner with another jurisdiction to complete all three interview types; or
  3. Request the State to conduct any of the three components on their behalf.

Please note that the survey will close at 6:00 PM on Saturday, May 30th. Your responses will be used to allocate funding through the CARS amendments for each jurisdiction.
