Reminder: Notice of Requirement for Digital Submission of Documents – effective April 14, 2020

This message is a reminder for DHS Division of Quality Assurance, Office of Plan Review and Inspection (OPRI) customers that in accordance with Governor Evers Emergency Order 22, all documents must be submitted electronically at this time.

The OPRI’s Milwaukee office is closed so digital submission of documents will enable ongoing plan review services during the COVID-19 crisis and improve safety for clients and staff. Please follow this process at this time:

  • Email all healthcare construction project documents to DHSDQAPlanReview@dhs.wisconsin.gov
  • Mail application and associated fees only to:

    DHS/Division of Quality Assurance
    Attn: Plan Intake Coordinator
    819 N. 6th St., Room 609B
    Milwaukee, WI 53203-1606

Unfortunately, OPRI is unable to process any health care construction project documents mailed or delivered to the Milwaukee office until the office is opened. Again, please email those project documents as described above to ensure timely processing. Plan review applications and associated fees received in the DHS/DQA mailbox via USPS will be processed specific to the digital submittals. Plan submittals will be processed upon receipt; however, conditional approval will only be granted upon receipt of associated fees. Inspection format and frequencies continue to be evaluated on a case-by-case basis.
