Misconduct Incident Reporting (MIR) system: IMPORTANT NOTICE regarding outage on Sunday, April 12, 2020

This notice is to alert all Division of Quality Assurance (DQA) regulated health care facilities and providers that the Misconduct Incident Reporting (MIR) system will be unavailable from 5 a.m.-3 p.m. on Sunday, April 12, 2020.

Due to a server upgrade, providers will not be able to submit Alleged Nursing Home Resident Mistreatment, Neglect and Abuse Report (F-62617) and/or Misconduct Incident Report (F-62447) forms using the MIR during this time.

Any reports that cannot be submitted while the system is unavailable should be submitted using the MIR system after 3 p.m. on Sunday, April 12, 2020.

DQA Regional Offices are aware of this system outage; the delayed submission of reports as a result of the system being unavailable is expected.
