Interim Process for IRIS 1-2 Bed Adult Family Home Providers During COVID-19

Due to the COVID-19 pandemic, the Wisconsin Department of Health Services (DHS) has defined an interim process for certification and recertification of IRIS 1-2 bed adult family home (AFH) providers. Please see the following important guidance, which is effective immediately and until further notice.

New Certification

Certification of new providers will be put on hold unless the provider has an IRIS program participant confirmed to live in the home. DHS will use an interim process to complete the certification, and will conduct a site visit after the emergency situation is over. If you have a participant confirmed for your AFH, please call 844-910-3658 to get a list of materials you must submit. When you call, you will be asked for the name of the participant and the IRIS consultant. Please do not send this information via email.

Annual Recertification

DHS will only recertify 1-2 bed AFHs that are active IRIS providers in good standing. You will be required to submit all required certification documentation with your application, as DHS will not be conducting on-site visits at this time.

Certification for Programs Other than IRIS

Applicants who will be serving participants in other programs such as Family Care should contact the managed care organization or certifying agency for that program to find out what AFH certification processes the agency is following during the COVID-19 pandemic.
