Revised SOD/POC Process Effective April 15th
Effective April 15, 2019, the Division of Quality Assurance, Behavioral Health Certification Section will email survey results to providers via administrative staff instead of directly from the surveyor who conducted the survey.
If a Plan of Correction is required, the email will direct you to return the completed plan to DHSDQAMentalHealthAODA@dhs.wisconsin.gov. All other aspects of the survey process will remain the same.
