This publication replaces DQA Memo 15-006 Certification of Waivers and Variances Submitted to the DQA Behavioral Health Certification Section. DQA Form F-60289 Waiver or Variance Request is required for use by certified behavioral health and community substance abuse service providers for each certification requesting a waiver or variance from the Behavioral Health Certification Section. Waiver or Variance Requests not submitted on DQA Form F-60289 Waiver or Variance Request will not be accepted.
