Join the Bureau of Assisted Living, Racine Transitional Coalition and local community members as we share a FREE educational session to help improve your transitional care process. Each attendee will be provided with transitional resources to help them start.
Four sessions are available for attendance:

January 16, 2019: St. Monica’s Senior Living
