Resident Relocation Manual

Under Wisconsin law, facilities relocating five or more residents must file a resident relocation plan with the Department of Health Services, Division of Medicaid Services. A facility must not begin discharge planning for any of its residents until the Division of Medicaid Services (DMS) has approved the facility's resident relocation plan.

To assist facilities in the relocation process, the DMS Bureau of Adult Quality & Oversight is pleased to announce that the Resident Relocation Plan and the Resident Roster and Individualized Relocation Plan forms have been developed and are ready for use.  The forms can also be found in the Resident Relocation Manual
