Nurse Aide Renewal Instructions After September 1, 2018

The Division of Quality Assurance (DQA), Office of Caregiver Quality (OCQ) oversees federal and state requirements for the Wisconsin Nurse Aide Registry/Nurse Aide Training and Competency Evaluation Program (NAR/NATCEP). On September 1, 2018, the management of the NAR/NATCEP will transition from Pearson VUE to D & S Diversified LLP dba as Headmaster. This message provides important information regarding the renewal process for nurse aide federal employment eligibility.

Pearson VUE is no longer accepting renewal forms for verification of nurse aide employment eligibility. Employers and nurse aides should send renewal forms through August 31, 2018 to:

DHS DQA Office of Caregiver Quality
1 West Wilson Street, Room 450
P.O. Box 2969
Madison, WI 53701

DHS will forward renewal forms to Headmaster and processing of renewals will resume on September 1, 2018. Headmaster utilizes TestMaster Universe (TMU©) online system for testing and registry services. To verify a nurse aide’s federal employment eligibility, employers of nurse aides need to:

  • Identify one or two individuals in your organization who will be responsible for verifying nurse aide federal employment eligibility.
  • After September 1, 2018, identified individuals must create an account with Headmaster by calling (888) 401-0465 (toll free).
  • Headmaster will provide a user name and password to log in to wi.tmuniverse.com
o   Click on the "Employment" menu option. This page lists all nurse aides that have requested verification of employment from your facility.
o   Click "Verify" if the employment information is correct, or "Deny" if the employment history is not correct.
  • Verification of the nurse aide’s employment will automatically renew the nurse aide’s federal employment eligibility on the WI Nurse Aide Registry for 2 years.

Note: An email will be sent to the email address of record for your facility each time a candidate requests an employment verification.  Entity individuals may also login to wi.tmuniverse.com at any time to see pending requests.
