Assisted Living and Hospital Interface Guide Now

The Division of Quality Assurance (DQA) is pleased to announced the release of our newest publication, the Assisted Living
and Hospital Interface guide. The DQA Bureau of Assisted Living (BAL) convened a work group to develop guidelines and offer tools to support transitions in care between assisted living facilities and hospitals/emergency departments. Standardization of transition in care coordination practices is critical since it integrates the different settings/practitioners systems/practices in improving the health care outcomes of the client being served; increases community based providers understanding of the clients’ medical conditions and proposed treatments; decreases emergency department visits, decrease in hospitalizations, readmissions and number of hospital days.
As part of the roll out of guide, Sarah Pyzyk, University of Wisconsin-Madison, School of Nursing, Doctorate of Nursing Practice Candidate, developed a self-assessment survey that Wisconsin assisted living facilities completed. Ms. Pyzyk has completed analyzing the self-assessments. The next steps are for her to share the results directly with individual assisted living facilities and provide training to you and your staff on the use of the guide.
