FYI/DHS Grant Funding Opportunity to Provide Multidimensional Family Therapy to Individuals Ages 16-25

The Wisconsin Department of Health Services (DHS) is seeking proposals for four organizations to provide multidimensional family therapy (MDFT) to individuals ages 16-25. Organizations must meet the following criteria in order to be considered for this grant funding opportunity.

  • Vendor is county-based, tribal-based, or nonprofit-based
  • Vendor is certified under DHS Chapter 75 as an Outpatient AODA Clinic. 
  • Vendor is certified under DHS Chapter 35 as an Outpatient Mental Health Clinic. 
  • Vendor has maintained continuous certification under DHS Chapter 75 as an Outpatient AODA Clinic and under DHS Chapter 35 as an Outpatient Mental Health Clinic for the past two years. 
  • Vendor is a certified Comprehensive Community Services (CCS) provider organization and is eligible to bill Medicaid under CCS or vendor will become a certified CCS provider organization within six months of award and vendor is eligible to bill Medicaid for the services outlined in the grant funding opportunity announcement.
  • Vendor has provided mental health services to individuals within the target age range of 16-25 within the last three years.
