regarding the Wisconsin
Nurse Aide Registry
Effective October 28, 2016, the use of the individual's birthdate is now a mandatory field when searching for an individual on the Wisconsin Nurse Aide Registry. Both the individual’s last name and birthdate must be entered for the most accurate results. You may enter up to 5 letters of the first name to narrow your search.
You may also search by the individual’s registration number and may search up to twelve numbers at once. The Wisconsin Nurse Aide Registry can be accessed at https://www.asisvcs.com/services/registry/search_generic.asp?CPCat=0750NURSE.
