CAREGIVER BACKGROUND CHECKS

Wisconsin State Statutes 50.065(3) requires the Department of Health Services conduct background checks on all entity owners, some board members, and non-client residents of Division of Quality Assurance (DQA) regulated entities as part of the licensure process and every four years thereafter.  In addition, entities are required to complete caregiver background checks on all new caregivers and at least every four years thereafter.

A completed background check consists of:
• A completed DHS form F-82064, Background Information Disclosure (BID);
• A response from the Department of Justice (DOJ)
• A “Response to Caregiver Background Check” letter containing information from the Department of Health Services (DHS), the Department of Children and Families (DCF) and the Department of Safety and Professional Services (DSPS).  

The “Response to the Caregiver Background Check” letter now includes information from the U.S. Department of Health and Human Services, Office of Inspector General (OIG) List of Excluded Individuals/Entities (LEIE) which provides information to the health care industry, patients and the public regarding individuals and entities currently excluded from participation in Medicare, Medicaid and all other Federal health care programs.  A name listed in this area may prohibit employment or licensure for the person.
