Wisconsin Department of Employee Trust Funds Employer What's New - Income Continuation Insurance Update

ICI Forms Now Available Directly From ETF

The Department of Employee Trust Funds (ETF) encourages employers to download and print all ETF-administered, employee benefit forms from the Employers Forms page at etf.wi.gov. When downloading is not an option, employers may order forms from ETF by clicking the Order Forms button under the Employer’s tab on etf.wi.gov. Income Continuation Insurance forms (formerly ordered from Aetna) are now available through these same channels. This should result in a streamlined process, speedy delivery to employers and cost savings.
