Questions? Please contact your program specialist.
For sponsors who wish to participate in the National School Lunch & Breakfast Program in SY20-21, a WINS renewal application and Meal Distribution Plan (MDP) Survey must be completed and submitted.
Program operators should fill out the MDP Survey based on current meal distribution methods. We understand during this public health emergency that school districts’ instructional and meal service models may change. Program operators should re-submit their MDP survey as their meal distribution models are updated.
Submitting Your MDP Survey
-
Print/ Download the MDP Sponsor Checklist.
- This checklist will guide you through the application and survey process by detailing the steps to take and what you must consider before submitting your MDP and Renewal Application.
- Sponsors do not need to return the MDP checklist to OSPI.
- We recommend gathering all of the required materials before starting your WINS application or MDP survey.
-
Submit Your MDP survey.
- Child Nutrition Services (CNS) automatically receives notifications when the survey is completed. For questions, please contact your program specialist or call 360-725-6200.
Due to the public health crisis, USDA Child Nutrition Program requirements are subject to change both as updated guidance is received from the USDA and at the discretion of the State Agency. Please check CNS Updates and OSPI's website for all important updates and/or changes.
