Apple Health (Medicaid) Provider Alert

The agency updated the January 1, 2018, published version of the Habilitative Services Billing Guide.

What has changed?

Effective for claims with dates of services on and after January 1, 2018, providers must use modifier 96 to identify services provided under the Habilitative Services program. Modifier SZ was previously used for this reason, but was discontinued on December 31, 2017.
