Apple Health (Medicaid) Provider Alert

The Ground Emergency Medical Transportation (GEMT) Provider Participation Agreement (HCA 42-0002) is available on the Health Care Authority’s (HCA’s) website.

What is the purpose of this agreement?

The purpose of the agreement is to let HCA know which qualifying publically owned or operated ground ambulance providers want to participate in the GEMT supplemental payment program.

When is the deadline?

You must complete, sign, and return the agreement to HCA no later than January 31, 2018. This deadline ensures that HCA has time to send you the data needed to complete your annual cost report (due March 31, 2018).

How long is the signed agreement effective?

The signed agreement is effective for one year.  A separate participation agreement is required for each year the provider would like to participate in the program. HCA is accepting agreements for state fiscal years 2016 through 2019.
