Providers, practices, and leaders in our state are
demonstrating their commitment to a healthier Washington by changing the way
health care is delivered and reimbursed.
To make this task easier, the Healthier Washington Practice
Transformation Support Hub and Qualis Health have created a Practice
Transformation Directory. With the directory, providers can find
information on a range of practice transformation opportunities available in
Washington and how to connect to the ones most relevant to their practice
goals. All programs share the common goal of helping practices achieve sustainable
improvements and patient outcomes.
The Hub is also partnering with the University of Washington
AIMS (Advancing Integrated Mental Health Solutions) Center to present a webinar
on financing integration of primary care and behavioral health. The webinar will
be Wednesday, July 19 from noon to 1 p.m. Register

Across the state, providers of physical and behavioral
health care are making the commitment to achieve true practice transformation.
They aren't expected to go it alone. Training and tools are available to help
them effectively coordinate care, develop clinical-community linkages, and
transition to value-based payment models.
The webinar, to be held from 1 to 2 p.m. July 6, will focus
on resources available to the provider community and some of the experiences
and tactics offered on the ground. Register

Healthier Washington invites public comment on the draft Project Plan Template and supplemental workbook, which
Accountable Communities of Health (ACHs) must use to submit their health system
transformation projects to the state under Initiative 1 of the Medicaid
Transformation Demonstration.
The Template and workbook help guide ACHs through the
project planning process, which requires detailed descriptions of their
selected transformation projects.

The draft Demonstration
Project Toolkit has been revised. The Toolkit is a guide for ACHs as they
complete their Project Plans. Changes to the Toolkit are based on feedback from
the Centers for Medicare and Medicaid Services (CMS) and the public. This
version includes more detail and clarity about milestones and timelines.
The Health Care Authority (HCA), in partnership with the
Department of Social and Health Services (DSHS), has selected Amerigroup as the
third-party administrator for the jointly managed Foundational Community
Supports program. The program is a critical part of Washington State’s Medicaid
Transformation Demonstration.
Amerigroup will work with HCA to manage the program that
provides supportive housing and supported employment services to eligible
people enrolled in Apple Health (Medicaid). Read the full announcement.
As a group, rural communities tend to be older and sicker
than residents in urban areas. At the same time, they have less access to
health care, which means their conditions are often diagnosed later with more
serious findings.
The Healthier Washington Rural Health Innovation Accelerator
Committee has produced a report, “A New Vision for Rural Health in Washington
State,” based on an assessment of the rural health landscape and a series of
convenings that included entrepreneurs and thought leaders from the public and
private sectors. Read the report.

The King County ACH has been engaged in a housing-health
partnership as a cross-sector innovation project that promotes health literacy
and access to care. Hear how important the program has become to the
participants and the positive outcomes produced by community health workers.
