Hub's Resource Portal launched today

The Practice Transformation Support Hub’s Resource Portal launched today. The Portal is an interactive platform designed to help primary and behavioral health practices across Washington State by:

  • Providing curated resources and vetted training materials to help with bi-directional primary and behavioral health integration, payment reform, and other issues.
  • Helping practices connect with local resources as part of a team-based approach to care.
  • Linking with the Accountable Communities of Health, local public health and other partners to address the unique needs of each region.      

The Portal is part of the Practice Transformation Support Hub, a Healthier Washington initiative managed by the Washington State Department of Health. The Portal is developed, curated and maintained by the University of Washington’s Department of Family Medicine Primary Care Innovation Lab (PCI-Lab).

The Portal is also aligned with the Hub’s coaching and regional connector services provided through Qualis Health. The web content includes the Population Health Planning Guide developed under the Plan for Improving Population Health initiative.

The Portal is a collaborative and ongoing effort between health care providers, professionals and advisors. Portal users and relevant content experts carefully vet each resource, creating a clearinghouse of practice transformation tools for healthcare practices.

This is just the first version of the Portal. New features and resources will be added regularly.
