Practice transformation support hub kick-off webinar January 10
The Practice Transformation Support Hub will launch its
official kick-off with a webinar from 12 to 1 p.m. Tuesday, January 10 on behavioral
health integration. The Hub, a critical component of the Healthier Washington
initiative, supports health systems transformation by providing resources,
tools, training and coaching services to help accelerate change of the health delivery system.
Behavioral health
integration webinar
Bi-Directional Health Integration: Improving Care for All
Patients, will be the first Hub learning event for 2017. The Hub will present
on how integration leads to better health outcomes for patients and benefits
providers, with case examples of successful integration, and other behavioral
health integration topics. Participants can register for free online.
Coaching and
connector services
Qualis Health—contractors for practice transformation
coaching and connector services—will offer providers and practices across the
state tools, training and hands-on technical assistance in behavioral and
physical health integration, value-based care, and improving population health
through clinical community linkages.
Coming soon: Hub
resource portal
In partnership with the University of Washington, the Hub
will launch a web-based resource portal at the end of January. The website will
be an interactive platform of curated resources designed to help practices in
Washington with practice transformation topics and issues.
