Apple Health (Medicaid): Provider Alert

Sterilization claims do not need to include the diagnosis codes Z98.51 or Z98.52. This was a mistake in the Sterilization Supplemental Billing Guide published on April 1, 2016. These codes are not to be used for the sterilization procedure. Please use correct coding procedures and continue to use the sterilization diagnosis code of Z30.2 for the sterilization procedure. The error will be corrected in a future edition of the Sterilization Supplemental Billing Guide.
