Deadline for starting an exemption to the vaccine mandate is Sept. 13

Hello ESD colleagues,

If you want to request an exemption to the COVID vaccination mandate, we’re asking that you start that process with HR by Sept. 13.

The reason why we’re setting this deadline is because the full process takes time, and we want to make sure we can complete it by the Oct. 18 deadline.

To start the religious exemption process, please:
Turn in the Religious Exemption Request form to ESDGPHRCOVIDResponseTeam@ESD.WA.GOV by Sept. 13.

To start the medical exemption process, please:

• Turn in the Vaccine Proclamation Medical Questionnaire signed by your doctor to ESDGPHRCOVIDResponseTeam@ESD.WA.GOV by Sept. 13.

            OR

• Send an email to ESDGPHRCOVIDResponseTeam@ESD.WA.GOV by Sept. 13 letting us know that you’ve scheduled an appointment with your doctor.

            OR

• Send an email to ESDGPHRCOVIDResponseTeam@ESD.WA.GOV by Sept. 13 letting us know that you’ve sent the Vaccine Proclamation Medical Questionnaire to your doctor.

Vaccine verification process
I’m sorry for the delay, but we’re still finalizing this process based on new guidance we’ve received. I’ll be able to update you soon.

I know many of you are anxious to get your vaccination status verified. I assure you that we’ll have a process deployed in time to verify all vaccinated employees before Oct. 18.
