Here are some resource flyers we have for those needing health insurance.

My Health Pays rewards- Value Added Benefit:

  • My Health Pays reward amounts and activities have changed – As of January 1, 2021

Apple Health Members can earn rewards for the following activities:

  • $15 - Complete your Initial Health Screening forms within 90 days of enrollment
  • $10 - Annual Flu Shot (Sept. 1-March 31)
  • $25 - Annual Well Visit - once per year, available for infants and up.
