New Mandatory Reporting Resource

There is a new, optional template on our Provider Resources page under Incident Reporting that provides contact information for reporting abuse, neglect, exploitation and abandonment. It also includes other important phone numbers that should be readily available to staff and clients.

The first page allows you to add phone numbers specific to each home/setting. The second page is a quick print form that can be cut into four, smaller postings to ensure people have access to the information on how to report abuse, neglect, exploitation and abandonment to Residential Care Services' Complaint Resolution Unit.
