Washington State COVID-19 Disability Survey
Several disability organizations in Washington are partnering to collect information about people’s experiences with finding and getting the COVID-19 vaccine. Please consider spending a few minutes taking this survey.
All responses are optional and anonymous. The information will be used to create outreach and partnership programs to increase vaccine awareness and access for people with disabilities and their families. The survey is available in multiple languages.
