Forms Required for Waiver Services
DSHS’ Developmental Disabilities Administration updated the following forms to reflect waiver amendments that went into effect October 1, 2020. The forms guide providers and assist with auditing purposes to ensure providers have a standardized way to report to DDA their approach to supporting their clients and provide progress updates.
Case resource managers must provide these forms (or the link) to providers working with a client under the specific service, prior to the service beginning.
Initial forms are necessary to set a baseline with a client within the first 30 days. Progress report forms are also necessary to update the CRM every 90 days. For more information, please see the DDA Policy Manual.
