Dear Supported Living, Group Home and Group Training Home providers,

A new and improved Residential Allowance Request form is now available. Requests using the old form will not be accepted after September 1, 2018. Refer to DDA Policy 6.11, Residential Allowance Requests, or contact your regional Resource Manager for questions.
